I think Tokuhiko Uwabo did this one, as it was not Kazuhiko Nagai or Chikako Kamatani.

Any evidence for this?

Tokuhiko did a lot of games for Sega back then. When I asked Kazuhiko, he said neither him or Chikako did it. I don't know 100% if it's Tokuhiko though since it's hard to get a hold of him.
